Care Management Coordinator

Under general direction the Care Management Coordinator (CMC) in conjunction with the Care Managers, coordinates care management services, conducts outreach calls to VPA or Managed Care patients and conducts initial Care Management screening.  The CMC may provide non-clinical support services including logistically coordinating hospital inpatient and ED discharge patient follow up.  Serves as an initial referral point for Care Management services for patients with complex needs. 

Responsibilities: Under general direction and according to established policies and procedures: 

  •  Manage a panel of patients by:

o    Initiating outbound calling sequence for patients recently discharged from the Hospital or after an ED visit.

o    Conducts initial patient screening based on prescribed assessment tools

o    Interprets customer needs and assist in arranging resources to achieve customer satisfaction

o    Follows up on non-clinical patient issues and refers cases to Care Managers based on patient needs

o    Maintain all patient and referral documentation within the Care Management software

o    Interacts as directed with all members of the care team to enhance care coordination

  •  Coordinate patient needs with Care Managers, local practice PCCs and schedulers
  •  Organize a variety of administrative and clinical tasks and prioritize in order of importance and impact on patients and providers
  •  Assist in preparation of daily, weekly or monthly reporting documents
  •  Contribute ideas for performance and process management improvement within the department
  •  Assist department in administrative functions
  •  Perform special projects as assigned

 Requirements  

  • High school graduate with some college level course work and or related experience in health care field or certification in medical assistance program
  • Experience in customer service
  • Self-directed with excellent problem solving skills.
  • Ability to multi-task while maintaining a professional demeanor 
  • Excellent verbal and written communication skills.
  •  Excellent computer skills
