The Wound Care Nurse is responsible for implementing clinical programs and best practices that govern the provision of wound care services within Pinnacle Senior Care. He/she works closely with branch locations to ensure the programs and protocols meet professional standards of care for quality compliance and reimbursement that comply with company policies and procedures as well as federal, state, and local laws, and regulations.


  • Collaborate with Wound Care Program Manager to ensure that wound management program standards, policies and materials comply with accepted ethical conduct, professional standards of care, along with state and federal regulations
  • Assist the Wound Care Program Manager in the planning, development, and implementation of the wound management services provided by the company
  • Participate in the development of clinical operational policies, procedures, and processes relating to wound management services provided by the company
  • Assess, plan, and provide location training with initiatives and education in wound care standards, documenting via a variety of means, including in-service training, practical labs, one-on-one teaching, and classroom teaching and home visits
  • Provide support to office locations with development and implementation of process improvement initiatives relative to wound management
  • Make recommendations for formulary inclusion
  • Review branch clinical and operational data and reports to monitor trends and outcomes relative to wound management
  • Identifying opportunities for improvement, also assist in the development of action plans and process improvement
  • Participate in local, state and national organizations, meetings, seminars, workshops and activities relating to wound management services in the home care industry, as appropriate
  • Provide direct input with each branch in the hiring process of other certified wound nurses or wound team case managers
  • Promote the interdisciplinary team approach to patient care
  • Other duties as assigned


  • Must be a Registered Nurse
  • 3-5 years of recent clinical experience as a registered nurse in a health care setting (i.e. acute hospital, SNF, rehab facility, outpatient clinic or home health)
  • High level of professional accountability and ability to function autonomously with minimal supervision
  • The ability to work flexible hours as required
  • Work closely with Visiting Physicians to form a cohesive wound care team
  • Absolute integrity and knowledge of HIPAA regulations, Stark laws and Medicare regulations
  • Must possess the disposition and ability to work harmoniously with a wide variety of people and all levels of staff while interacting in a professional manner as a member of an interdisciplinary team
  • Strong clinical assessment and case management skills
  • Excellent organizational, time management, interpersonal, communication skills
  • The ability to effectively present to large groups
  • Possess an active unencumbered license as a registered nurse in the state in which he/she is required to practice
  • The ability to travel is necessary
  • Certification by National Alliance of Wound Care (WCC)
  • Home Health experience
