VPA serves the elderly and other adult patients who are suffering from illness or chronic conditions in the safety, privacy and comfort of their home. Our physicians collaborate with home health and hospice agencies to establish in-home treatment plans and certify the need for services.

POSITION DESCRIPTION

Deliver primary health care and patient care management in a residential setting and facilities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Take history, examine, determine diagnoses, and treat scheduled patients.
  • Document patient's visit, per NCQA standards.
  • Perform phlebotomy (venipuncture only), as necessary.  Perform injections and/or immunizations. May perform ECG, Holter and/or Pulse Oximetry occasionally.
  • Provide patient education.
  • Perform minor surgeries; I&D, removal of foreign body, suture removal, minor lacerations, minor aspirations.
  • Work with Patient Care Coordinator to assure orders and referrals are properly/timely placed.
  • Communicate with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis and treatment.
  • Perform all clinical duties while observing OSHA Universal Precautions.
  • Maintain patient confidentiality.
  • Attend required meetings and in-services and participate in committees, as requested.
  • Participate in professional development activities and maintain professional licenses and affiliations.
  • Perform related work as required

REQUIRED KNOWLEDGE, SKILLS AND EXPERIENCE

  • Experience with Geriatric or complex patients preferred
  • Active/unrestricted medical license and DEA
  • EMR experience
