POSITION DESCRIPTION  

Visiting Physicians Association (VPA) is actively recruiting for a Patient Care Coordinator (PCC). This is a unique position because it allows the PCC to support the Provider in taking care of the patients, orients and educates patients and their families by meeting them; explaining the role of the patient care coordinator; initiating the care plan; providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements. As a PCC, you are communication bridge between the patient and the provder.

VPA a national leader in delivering home based primary care, works with home health and hospice agencies to ensure patients receive continuity of care in the home environment. VPA in-home physicians collaborate with agencies to establish in-home treatment plans and certify the need for services. VPA partners with over 1,000 home health agencies, collaborates with independent and assisted living communities, and works with skilled nursing facilities and hospitals nationally to coordinate services and patient transitions to home care.

ESSENTIAL DUTIES AND RESPONSIBILITIES  

  • Responsible as the point person for the Provider’s patients, facilities, and other healthcare professionals
  • Answer calls and collect all pertinent medical information needed for the providers medical decision making
  • Process Provider orders
  • Schedule consult and specialist appointments
  • Request & collect medical records
  • Document all calls in EMR
  • Perform continuous detailed tasks with frequent interruptions
  • Collaborate with other divisions within Care Center
  • Review and assign faxes and attachments
  • Provide support to Care Center RN when triage is needed and urgency is escalated
  • Ability to work as a team to cover other PCCs without being asked
  • Ability to develop strong rapport over the phone
  • Maintains patient confidentiality and all HIPPA privacy requirements
  • Attends required meetings and in-services
  • Ability to adapt and apply guidelines and procedures
  • Strong customer service approach to all callers
  • Performs related work as required

REQUIRED KNOWLEDGE, SKILLS, AND EXPERIENCE 

  • LPN certification (Required)
  • Minimum 2 years office experience in health care coordination
  • Strong medical office call experience with ability to collect appropriate medical information from the patient/caller to communicate to provider
  • Basic working knowledge of medical terminology
  • Computer skills: Microsoft Excel, Word, Outlook
  • Strong customer service, organizational and interpersonal skills with excellent multi-tasking and prioritization 
  • Professional demeanor, self-motivated, and dependable 
  • Ability to communicate effectively with all personnel levels within the organization
  • Ability to work closely with Patient Care Center RN and Provider
  • Strong phone communication skills

PREFERRED KNOWLEDGE, SKILLS, AND EXPERIENCE  

  • Certified (AAMA) MA, EMT, Military Medic, Paramedic, Foreign Nursing Grad, or Foreign Medical Grad  
  • Experience in EMR strongly preferred

PHYSICAL REQUIREMENTS/WORKING CONDITIONS

  • Full range body motion including handling/lifting patients
  • Ability to drive up to 150 miles daily in urban and/or rural settings
  • Ability to lift/carry items weighing up to 50 lbs
  • Ability to read and understand road maps
  • Sit or stand for long periods of time
