Visiting Physicians Association (VPA) is growing and searching for Palliative Care Nurse Practitioner's to join the team! We are hiring immediately in the Columbus, OH and San Antonio, TX areas and will be searching for Practitioners in additional states in 2019. Our NP's will provide palliative care treatment and consultation to chronically ill patients under the supervision of one or more physicians to optimize the quality of life. The nurse practitioner is an extension of the physician being able to provide full palliative care across all patient types. The Nurse Practitioner is able to prescribe therapy or medication, order and interpret tests and develop comprehensive treatment plans in cooperation with the patient’s primary care provider.

Essential Duties and responsibilities

•Serves as a comprehensive direct care provider with demonstrated skilled proficiency in symptom management and advanced illness care
•Provide comprehensive clinical and psychosocial evaluations of patients and their families in relation to their quality of life goals
•Serves as a patient advocate in the development of patient-centered plans of care
•Collaborate with patients and families to establish goals of treatment and advanced directive options
•Collaborate with primary care to ensure interdisciplinary communication as well as treatment goals
•Open and responsive to diverse backgrounds and experience of other people and promotes an environment that is sensitive to cultural diversity
•Understands and is able to discern continuum support services as adjunct assistance for patient stability
•Act as an advocate for USMM continuum of care in the community
•Demonstrated knowledge of Medicare benefits, patient rights, advance directives, and community resources
•Demonstrate proficiency in concurrent EMR documentation and appropriate coding

REQUIRED Knowledge, Skills and Experience

•Advanced Practice Nurse with appropriate active and unrestricted state licensure
•Valid NPI#, DEA and CTP
•Enrolled with Medicare or state Medicaid or eligible for enrollment
•3 years of home-based or hospice care or relevant experience
•Demonstrates excellent observation, verbal and written communication, problem-solving, and organizational skills

Visiting Physicians Association, a national leader in delivering home-based primary care, works with home health and hospice agencies to ensure patients receive continuity of care in the home environment. VPA in-home physicians collaborate with agencies to establish in-home treatment plans and certify the need for services. VPA partners with over 1,000 home health agencies, collaborate with independent and assisted living communities, and works with skilled nursing facilities and hospitals nationally to coordinate services and patient transitions to home care.
